Dans cette adaptation libre de la pièce « Henry IV » de Shakespeare, Mike Waters (River Phoenix) est un prostitué homosexuel souffrant de narcolepsie. Scott Favor (Keanu Reeves) est le fils rebelle d’un maire. Ensemble, ils voyagent de Portland, dans l’Oregon, jusqu’en Idaho, puis jusqu’aux côtes italiennes, dans l’espoir de retrouver la mère de Mike, dont il est séparé. En chemin, ils se prostituent pour gagner de l'argent et se procurer de la drogue, attirant finalement l'attention d'un riche bienfaiteur aux penchants sexuels déviants.
